Telehealth technology allows us to monitor conditions when our staff is not present. If your home health care plan indicates the use of telehealth, you will have a monitor set up in your home to track vital signs such as blood pressure, heart rate and weight. This monitor will not only alert you when it is time to check these vital signs, but will be programmed to ask questions about your specific health condition. A nurse in our office reviews this information and calls you with any noted changes. These changes may indicate a need to have one of our nurses visit you or call your doctor.
