Geriatric Assessment Center
Whether an older adult is coping with challenges resulting from physical, mental or emotional changes, the Geriatric Assessment Center at Lancaster General Health Campus can help identify the most appropriate solutions. The assessment includes a comprehensive evaluation by a team of healthcare professionals that specializes in issues related to aging. A specific recommendation for care is then shared with the family doctor and the family. Individuals and family members faced with any of the following situations would particularly benefit from an assessment: complex medical issues, memory impairment, multiple hospitalizations, social situations that impact health and consideration of nursing home or other retirement living arrangements. Today more and more people are receiving advanced healthcare services right in their own homes. Nearly 70 percent of the home healthcare visits provided last year by the Visiting Nurse Association of Lancaster County were to people 65 and over.
