In 2005, Roseville Pediatrics implemented the Medical Home program. Medical Home is a philosophy within our practice to provide care coordination for Children and Youth with Special Healthcare Needs (CYSHCN). Care coordination involves several components such as patient care plans, parent partners, connecting families with community resources, parent advisors for other families in our practice, focus groups, along with various other aspects. Please take a moment to look at our Medical Home bulletin board when you come to our North Pointe office. You’ll find information on upcoming conferences, summer camps and several community resources.

 Parents are an essential component of Medical Home. At Roseville Pediatrics, we are always looking for new parent partners to join our Medical Home team. Another role for parents within our Medical Home program is parent advisors.
