Heart Failure
Heart Failure Treatment
(The higher the percentage, the better the result)
Discharge Instructions
Heart failure patients discharged home with written instructions or educational material given to patient or caregiver addressing all of the following: activity level, diet, medications, follow-up appointment
Eval LV Function
Heart failure patients who had a test performed to evaluate their left ventricular (LV) function before arrival, during hospitalization, or is planned for after discharge
ACEI or ARB for LVSD
Heart failure patients with lowered heart function (left ventricular systolic dysfunction, or LVSD) who are prescribed medications (either angiotensin converting enzyme inhibitor, or ACEI, and angiotensin receptor blocker, or ARB) at hospital discharge
Smoking Cessation
Heart failure patients with a history of smoking cigarettes, who are given smoking cessation advice or counseling during hospital stay. For purposes of this measure, a smoker is defined as someone who has smoked cigarettes anytime during the year prior to admission
