Note:  This summary of benefits provided by LGH is a brief reference and not intended to be a complete list of benefits. 

PAID TIME BANK (PTB): The Paid Time Bank provides compensation for short term or intermittent absences of a personal nature.  It is a combination of 6 holidays, 4 personal days, 4 sick days and 10 or more vacation days, depending on length of service.  There is complete flexibility in using the time as needed. 

When Eligible:  Accrual begins immediately. Access is immediate for scheduled time and after three months for unscheduled time.  Scheduling is per department policy.

Part-Time Employees: The number of PTB hours earned is determined by multiplying the number of hours worked in each 2 week pay period by the PTB factor which applies to your years of service (example: if you have been here less than 5 years and worked 40 hours in a 2 week pay period, you would multiply 40 x .093 = 3.68 hours earned into your Paid Time Bank). The PTB factor varies with years of service.

HEALTH INSURANCE: Eligibility after one month

Full-time and part-time employees may choose from among three medical plans.  Premium costs vary with the plan, level of coverage, and your status, and will be deducted from each pay before taxes.  All health plans offer a prescription benefit.  Part time employees with assigned hours of at least 32 hours per week (.8 FTE) pay the same premium as full-time employees.

DENTAL INSURANCE: Eligibility after one month

Preventive and simple restorative dental procedures are paid under a schedule with major restorative procedures being paid at 50%.  The plan pays up to a maximum of $1,000 per calendar year per individual.  Premiums are deducted from each pay before taxes.  Premium rates are in the Benefits packet. Deductibles are $25 per single person and have a $50 maximum per family. Part time employees with assigned hours of at least 32 hours per week (.8 FTE) pay the same premium as full-time employees.

SHORT TERM DISABILITY(STD):  Lancaster General provides employees with STD coverage at no cost. In the event you become seriously ill or disabled it provides 100% of your income for up to the first 60 scheduled work days or 90 calendar days.  The benefit begins after the first 7 consecutive calendar days following the onset of illness. The number of benefit days or hours available depends on the how much time the employee has accumulated prior to the onset of the illness. 

When Eligible:  Accrual begins immediately but you cannot use your STD until after 3 months of continuous employment.  Regular, full-time employees accrue 64 hours/year (8 days) into Short Term Disability (STD) accrual.  The maximum accrual is 480 hours.

Part-Time Employees:  The number of STD hours earned is determined by multiplying the number of hours worked in each 2 week pay period by .031 (example: if you worked 40 hours in a 2 week pay period, you would multiply 40 x .031 = 1.24 hours earned into Short Term Disability accrual).  The maximum accrual is 480 hours.

Unlimited Short Term Disability (STD): Employees who accumulate 480 STD hours will be awarded Unlimited STD.  This means there is no need to replenish the STD hours they are used.  Unlimited STD will be available for as long as the person remains employed by the Lancaster General health system.  (STD applies only to the first 90 consecutive calendars days of a personal illness and LTD applies to eligible employees after 90 days.)

LONG TERM DISABILITY (LTD):  Eligibility:  After one year of service for employees assigned to work 32 hours or more per week (.8 FTE). Lancaster General provides employees with LTD coverage at no cost. In the event you become disabled it provides 60% of your pay up to age 65, up to a maximum of $7,000 monthly.  The benefit begins with the 91st day of disability.
PENSION:  Eligibility is at age 21 after at least one calendar year of service working a minimum of 1,000 hours per calendar year. Participants with 5 years of service with at least 1,000 hours per year are vested.  The benefit is equal to 1.25% of your average monthly salary times your credited vested years of service.  See the Summary Plan Description (SPD) for more information.

LIFE INSURANCE:  Eligibility after one year of service
Lancaster General provides employees with coverage at one times their annual earnings plus an additional $2,000 not to exceed $300,000.  Accidental Death & Dismemberment coverage is one times annual earnings or as scheduled.

VOLUNTARY LIFE INSURANCE  PLANS & ACCIDENTAL DEATH AND DISMEMBERMENT:
Eligibility is immediate. You can purchase additional life and AD&D insurance for yourself and your dependents at low group insurance rates.  You can sign up for these benefits at any time during your employment. Life insurance(s) are subject to evidence of insurability.

VOLUNTARY SHORT TERM DISABILITY:  Eligibility after one month
Employees have the option to purchase disability income insurance.  This benefit helps to safe guard income in the event of a prolonged sickness or injury when you have little or no STD time accrued.  There is a one year pre-existing condition clause attached with 50% of benefit paid.

SHIFT & WEEKEND DIFFERENTIAL: Eligibility is immediate
A shift differential of 10% of base salary or minimum of $1 per hour is paid for 2nd & 3rd shifts. An additional 15% of base salary or a minimum of $1.25 per hour is paid as a 1st shift weekend differential.  A combination of 10% and 15% (25%) or a minimum of $2.25 per hour is paid on 2nd and 3rd shift on the weekends.

FLEXIBLE SPENDING ACCOUNTS:  To participate, you must be hired  before December 1 to enroll in the following January 1, and  work a minimum of 16 hours per week. This program allows you to use pretax dollars deducted from your pay in equal amounts to help cover eligible out of pocket medical/dental expenses and Day Care/Adult Care costs.

TAX SHELTERED ANNUITY:  Eligibility is immediate

A TSA is an option to save for retirement with pretax dollars and earn sheltered interest at favorable rates. 

TUITION ASSISTANCE:  Eligibility after six months

Full-time employees receive reimbursement of tuition costs for C or better for undergraduate courses, B or better for graduate courses. Part-time employees receive prorated reimbursement based on hours worked. Authorization is required in advance of starting classes. Maximum dollar limits per credit applicable.

FAMILY & MEDICAL LEAVE:  Eligibility is upon completion of one year or service and working 1,040 hours.

Family and medical leave is provided by the Family & Medical Leave Act of 1993.

FUNERAL LEAVE:  Eligibility is immediate
Full-time and part-time employees are eligible for leave with pay up to 5 days for immediate family, 3 days for parents/siblings, and 1 day for grandparent or grandchild.  See your employee handbook for definitions of immediate family.

JURY DUTY & MILITARY LEAVE: Eligibility is immediate
Full-time and part-time employees will receive full pay for jury duty on days scheduled to work.  A leave without pay will be granted for military duty in accordance with federal regulations.

PHARMACY:   Employees are given discounts on prescription drugs as well as a discount on over-the-counter items through the LGH Convenience Pharmacy.

VISION CARE:  Eligibility is immediate.  Discounted services, frames and lenses are available at specific locations.  Refer to your benefits packet.

EMPLOYEE ASSISTANCE PROGRAM:  Eligibility is immediate.  Confidential counseling services to help you and your family.

ADDITIONAL DISCOUNTS & BENEFITS:  Eligibility is immediate
We offer a variety of other voluntary benefits such as personal cancer insurance through AFLAC, supplemental disability income insurance through Mass Mutual Life Insurance Company.  Also, free parking, discounted meals in the LGH cafeteria, payroll deductions, free flu shots, and inoculations.  The Employee Service Center is available on the first floor of the hospital . The Identification Badge machine is located in the ESC. Services available are dry cleaning, film processing, use of fax machine, copier and PC’s with internet access, discounted movie tickets, special show tickets, amusement park tickets and other coupons.
