The Neurological Day Treatment Program places a heavy emphasis on increasing independence at home, in the community and in school or workplace.
Home
If your main goal is living independently at your home or staying alone while your caregiver continues to work, the team will work with you and your family on determining your needs to safely reach this goal. They will provide recommendations for adaptations to your environment if necessary and will educate you and your family on strategies that you can carry over at home. The team can also advise you on community resources that are available to help you function at home and in your community at your highest possible level.
Work
If you worked prior to your injury, then any skills necessary to return to this work will be part of your treatment plan. The Case Manager and/or Office of Vocational Rehabilitation Counselor will obtain a job description and work with you, the team, your doctor and your employer to complete an assessment and make recommendations regarding the possibility of returning to previous employment. They will immediately begin integrating these goals into your rehabilitation program at a level that is appropriate for you based upon your recovery. When you are ready, the team will work with you and your employer to perform on-site visit and job trials if necessary.
The team will advise you and your employer of any adaptations that may be needed at the job site and the Office of Vocational Rehabilitation Counselor will assist in job reentry arrangements. If return to your same position is judged unrealistic given your level of functioning, the vocational counselor and the team will work with you and your family to advise you of available resources in the community or educationally to assist you in finding an appropriate position or alternative.
School
If you were attending school prior to your injury, any skills necessary to return will be a part of your treatment plan. The Case Manager will contact your school to obtain records, determine needs, coordinate your care and facilitate reentry. Homebound teachers may be called in to tutor during the rehabilitation process or until actual school reentry is possible. The team will work with you and your school on modifying the school environment and program to meet your individual needs and allow you to use strategies that have proven beneficial.
