TEAM APPROACH
The Neurological Day Treatment Program takes a very holistic approach to recovery from illness. Utilizing a team of professionals, we work with patients and their families to develop a program schedule and plan of care which best meets their needs and goals. The team meets regularly to coordinate the patient’s care and facilitate progress.
Each patient will have a case manager who will communicate progress or necessary changes in the plan and goals to the patient, family, physicians and payer sources. In our program, this Case Manager is also a Registered Nurse who can assist in providing any appropriate medical interventions or education.

In addition to your individualized treatment sessions, you may also be scheduled to participate in group programs. Group Programs address such issues as adjustment to illness, community re-entry activities, home and social skills, health education and maintenance.
Length of stay in the program is dependent upon individual rehabilitation needs and progress in treatment. Within two weeks of the initiation of services, an Initial Team/Family Conference will be held and a tentative discharge date will be communicated. The average length of stay is about 4-6 weeks but can be adjusted if indicated.
