WHAT TO EXPECT ON THE FIRST DAY AND GENERAL PROGRAM INFORMATION
What to bring
1. Insurance Cards and claim forms if applicable.
2. Any equipment you need to function such as wheelchair, walker, brace,
etc.
3. Any medications or self-care items that are needed during the hours of
the program. Please bring the medication in a container labeled with
your name, dose and time to be taken.
4. Any physician orders related to the Program.
5. A list of your medications and allergies.
6. A list of any upcoming medical appointments.
7. Two emergency contact telephone numbers.
8. An extra set of clothes is sometimes helpful and can be kept at the
facility at your request.
9. Please do not bring any valuables except for a wristwatch and a small
sum of money for any minor purchases.
10. If diabetic, please bring your own snacks and glucometer to use if
needed.
What to wear
1. Comfortable, loose fitting clothing
2. Sensible, supportive shoes. No slippers please!
3. Any splint, sling or brace that was issued to you
What to expect on your first day
1. You will come into the main entrance of the Suburban Outpatient Pavilion
at 2100 Harrisburg Pike.
2. There is a turn around in front for drop-off prior to parking. Valet
parking is also available at this location at no charge.
3. The Information Desk will provide directions to the Physical
Rehabilitation Center on the first floor and can assist in obtaining a
wheelchair if necessary.
4. Once you reach the Physical Rehabilitation Center, report to the
receptionist stating that you are here for a new evaluation for the Neuro
Day Treatment Program. The receptionist will register you and notify
the Case Manager of your arrival.
5. The Case Manager or another staff member will give you a tour of the
facility.
6. The Case Manager/Nurse will conduct the admission intake evaluation
and provide you with your schedule and necessary materials and
information.
7. Each Team Member will be introduced as you flow through your day.
8. At the end of the day, the staff will assist you in gathering your
belongings. Whoever has been designated to transport you home can
pick you up at a pre-arranged location within the Health Campus at their
convenience. If you have been advised to be supervised for your safety, a
staff member will be available.
Meals
1. Lunch will be provided by the Program in different formats to give you
“real-life” experiences. However, on some days, we ask that you practice
your skills at home and bring a brown bag lunch labeled with your name.
The Case Manager and staff will notify and remind you of when a bag
lunch is required.
2. If you have certain preferences, there is a refrigerator available for items
you wish to bring. This includes any individual snacks that are within
your dietary restrictions. Again, please label these with your name.
3. On community outings away from the Health Campus patients may be
responsible for paying for their own personal purchases which may
include lunch.
