Individual Nutrition and/or Diabetes Consultation:

• Designed for people who are unable to attend the comprehensive group classes or need additional education in nutrition/diabetes care.
• Registered dietician provides information on diabetes meal plan and related nutrition topics.
• Registered nurse provides information on diabetes facts, medications, hypoglycemia/hyperglycemia, monitoring of blood sugar, and foot care/long-term complications.
• Reimbursable by some major medical insurance/HMO's
• Referral from your physician may be needed for reimbursement
• Must be scheduled through the Diabetes & Nutrition Center at (717)544-5923
